Dr. Jeffrey Forman and Lou Ellen Bass will be our guest speakers giving us an update on COVID-19 based upon real experiences.
Lou Ellen Bass has been in nursing for 28 years. She was born & raised in Suffolk. She graduated from Suffolk High School & earned her LPN from Louise Obici School of Practical Nursing. Lou Ellen worked as an LPN on the Medical-Surgical & Pediatric units of Louise Obici Hospital and at Lakeview Medical Center where she assisted Dr. Jeffrey Forman in the Internal Medicine & Pulmonology clinics. After taking 7 years off to be a mom of 3, Lou Ellen graduated from Paul D. Camp Community College with her Associates Degree in Nursing. She worked as a Registered Nurse in the Intensive Care Unit of Sentara Obici Hospital and an Infusion Therapy Nurse with Sentara Home Care. For the last 6 years, Lou Ellen has served as the RN Practice Coordinator of Sentara Family Medicine Suffolk. She has since obtained her Bachelors’ in Nursing from Old Dominion University and is Board Certified in Ambulatory Care.
Lou Ellen lives in Suffolk with her husband of (almost) 30 years, Brian Bass. They have a daughter, Hannah & a son, Andrew & a son Jared (who is married to Brooke.)
Lou Ellen enjoys the beach, fitness, reading & spending time with family.
Dr. Jeffrey Forman has been practicing medicine in Suffolk for the last 25 years. He was born & raised in Baltimore, Maryland.
He graduated Magna Cum Laude with a degree in Biochemical Sciences from Harvard University, he obtained his Medical degree from Hahnemann University (now Drexel) School of Medicine & completed his residency & fellowship at Johns Hopkins. Since then he has gone on to obtain his Master of Sciences in Health Care Delivery from Dartmouth College.
He is triple board certified in Internal Medicine, Pulmonology & Critical Care Medicine.
Currently Jeff serves as the Chief Medical Officer of Population Health & Director of Hospital Based Medicine for Bayview Physicians Group. He is the Director of the Transitional Care Clinic at Chesapeake Regional Medical Center & the Director of the Lung Cancer Screening Clinic at Lakeview Medical Center.
He has hospital privileges at Sentara Obici Hospital and Chesapeake Regional Medical Center while providing care in the outpatient setting with Bayview Physicians Group at Lakeview Medical Center.
Jeff lives in Suffolk & has been married to Tricia Forman for 25 years. He has a son, David and a daughter, Maria. In his free time, Jeff enjoys Beekeeping, backpacking, kayaking, gardening and gourmet cooking,
